FS Visitation, Participation Agreement

Please download, complete, and sign for those who will be coming for your scheduled visit. Parent(s) and child(ren) can be on one form if they have the same parent/guardian.  Adults, 18 years or older, who are visiting, participating, or coming to watch someone MUST complete their own form and bring it with them. All areas that need completion will be highlighted in yellow on page 1, page 4, and page 5.
